The assessment is for you and the Coach to:

    Service Agreement

    You and the Coach together will review the Terms of Service and the policies before signing the Service Agreement.

    The Service Agreement includes:

    • Contact points
    • Program goals
    • Timeframe
    • Quote
    • Consent
    • Signatures

    During the appointments

    • Teamwork is encouraged
    • Policies and documents are reviewed together
    • You can exercise your rights and responsibilities
    • You can make informed decisions
    • You can ask questions to help you understand everything
    • Your support people are welcome to be involved and to assist you when necessary
