Waiver & Release

The Waiver and Release is an agreement between you and Deaf Chef Ross

Deaf Chef Ross provides Auslan accessible content that includes

• reviews of health and nutrition research
• general and individualised advice and recommendations
• suggestions for recipes
• meal planning ideas.

The aim is to help people make healthier choices.

We can assist you in understanding the advice given to you by your doctor or nutritionist.

This accessible content is for educational and informative purposes only. It is not to be used as medical advice or treatment for existing illnesses.

Our content is developed with the intention of educating and informing the general public and individuals about how nutrition can improve general health and wellbeing through Auslan.

When you sign this Waiver and Release Agreement between Deaf Chef Ross and yourself as a new client, that you are:

Acknowledging you are aware that Deaf Chef Ross is not a medical doctor, a qualified physician or licensed medical practitioner.

Deaf Chef Ross cannot diagnose disease or illness. If you have any health concerns, please see your doctor or GP.

Recommendations made by Deaf Chef Ross are recommendations only. Any changes you make to your diet are your choices and your responsibility.

You understand accept there is a risk involved with health, food and diet. You accept responsibility for the choices you make.

You are aware that Deaf Chef Ross is not a medical doctor, a qualified physician or licensed medical practitioner.

It is your responsibility to seek the advice of a qualified professional such as a doctor or nutritionist before you begin our program/new meal plan.

You are aware that some foods can cause an allergic reaction in some people.

You know that sometimes such allergic reactions can be serious or fatal. For example, nut allergies are common and differ in severity.

Deaf Chef Ross Cannot diagnose allergies. See your GP for an allergy diagnosis if you suspect you have one.

You have made Deaf Chef Ross aware of all allergies you knowingly have on the Deaf Chef Ross Client Information Form.

You are aware that specific foods may interact with certain medications.

You have discussed the possible side effects of all your medications with your doctor or pharmacist.

By participating in our diet/healthy eating program you accept all risks.

Should any injury, death or property damage occur due to my participation, Deaf Chef Ross is released from any liability.

Deaf Chef Ross cannot be held liable by my personal representatives, family, estate or next of kin.

You can authorise another person to act and sign on your behalf. This might be an advocate or a family member. It must be someone you trust.

Parents or Guardians will sign if you are under 18 years of age.

The participant and the person you have nominated to act on your behalf, agree you have:

1. Read and understand the Waiver and Release document
2. Have signed this Waiver and Release Agreement voluntarily.

By signing the Waiver and Release Agreement;

You agree you have carefully read the agreement (watched this video) and understand it to release Deaf Chef Ross from any claim for injury, death or damage to my property that occurs while participating in diet and healthy eating program.

If you understand and agree, you are now able to sign.

If you are uncertain, object or disagree please feel free to ask any questions you have for clarification. However, should you feel you are still unable to sign, Deaf Chef Ross will be unable to provide you with service.
